I recently had the opportunity to try the Bouquet Speculum, a device of recent design and clinical validation, in complex procedures such as cervical conization in patients with precancerous cervical lesions and transvaginal chorionic villus sampling in pregnancies with fetuses affected by genetic syndromes. The experience has been positive, highlighting its features that improve both patient comfort and clinical efficacy.
The Bouquet Speculum has proven to be significantly more comfortable for patients compared to traditional speculums. Its ergonomic design and materials allow for smoother insertion and positioning, which reduces the sensation of discomfort during the procedure. This feature is especially relevant in prolonged interventions or those involving delicate manipulation of the cervix and adjacent structures.
One of the most notable aspects of the Bouquet Speculum is its ability to provide an improved field of view. Unlike conventional speculums, this device offers a wider and clearer visualization of the cervical area, facilitating lesion identification and optimizing precision during procedures. This benefit translates into shorter intervention times and better guidance for the professional, potentially reducing the need for adjustments or repeat procedures.
The combination of greater comfort and better visibility directly reflects in the quality of clinical outcomes. In my experience, using the Bouquet Speculum has allowed for more precise cervical conizations, ensuring complete lesion removal without causing unnecessary damage to the surrounding tissue. Additionally, although it is not specifically designed for transvaginal chorionic villus sampling, I have successfully used it for this purpose, proving to be very effective in optimizing the procedure and minimizing the risk of complications.
While the Bouquet Speculum has proven to be an effective device, one area for improvement I consider relevant is the noise it generates when opening. This sound could induce pain-related anxiety in the patient, increasing stress during the procedure. Reducing or eliminating this noise would contribute to a better patient experience by minimizing procedure-related stress.
· Cervical Conization: During the removal of precancerous cervical lesions, the Bouquet Speculum has facilitated a more direct and secure access to the affected areas, allowing for a more efficient intervention with less discomfort for the patient.
· Transvaginal Chorionic Villus Sampling: Despite not being specifically designed for this use, I have successfully employed it for sampling chorionic villi, achieving a more manageable procedure that is less traumatic for both the mother and fetus.
In summary, the Bouquet Speculum represents a significant innovation in the field of gynecology and obstetrics. Its enhanced features in terms of comfort, visibility, and clinical handling offer substantial advantages that can benefit both patients and professionals. The adoption of this device in daily practice could significantly improve the outcomes of complex gynecological procedures and the overall patient experience.
Our team has found the Bouquet Speculum to be an invaluable tool in our Task Shifting program. The design and functionality of the speculum have made it significantly easier for our nurses to perform cervical screenings effectively and efficiently. The radial opening mechanism allows for better visualization of the cervix, because the lateral walls of the cervix do not fall into the way of view. This has been crucial for accurate assessments, and the ergonomic design minimizes discomfort for patients, leading to a more positive experience overall.
We have received positive feedback from the clinicians using the speculum, who appreciate its ease of use compared to traditional options. This aligns perfectly with our goal of empowering novice professionals to take on cervical screening responsibilities at the local level. The bouquet specula supplied for this initial phase have been instrumental in supporting our initiative, and we are optimistic about the impact they will have on cervical health in the community.
Our project has been using the Bouquet Speculum for cervical examination especially cervical health screening and we are amazed by the fact that its so easy to use with just lubrication, insertion and dilation giving you the best cervical exposure compared to the two bladed speculum. The Bouquet Speculum gives an excellent arrest of vaginal walls, limiting vaults prolapse, capturing a nice full circle cervical image and anatomy of the cervix.
Unlike ordinary two bladed speculum that you keep adjusting all around to get a cervical image, The Bouquet Speculum makes it easy for us. Just a one single insertion and a dilation of the vaginal walls giving the best patient comfort without tissue biting and multiple adjustments. This therefore makes it the best ever speculum for use here in Kenya and internationally. We highly recommend it to anyone. One single design, multiple functionality at its best.
Grace Gardens Hospital Ndop launched Cervical Cancer and Breast Cancer Screening and Treatment in July of 2021 using the World Health Organization approach of see and treat. During this Campaign at Grace Gardens, the provider used a 2-bladed speculum to access the cervix during which images of the cervix and tissue samples were taken.
After this first initiative, we shared the results of our campaign on social media. Dr. Jean Bouquet became interested in supporting our initiative and he had a zoom meeting with me during he introduced the Bouquet speculum. Without wasting time, Dr. Bouquet shipped some of the Bouquet speculum to our Faith Gardens Medical Foundation Headquarter in Dallas, Texas. These specula were then transported to Grace Gardens Hospital Ndop in Cameroon.
In June of 2022, we had our first experience using the Bouquet speculum. For three days, we screened 75 patients using the Bouquet speculum. The Bouquet Speculum being a novel, 5-petaled vaginal speculum that overcomes the limitations of the existing 2-bladed speculum is very user friendly. In the past when were still using the 2-bladed speculum, we will improvise by adapting the 2-bladed speculum using gloves which will help prevent the collapse of the lateral walls. The current vaginal speculum, in some cases, does not allow for complete visualization of the cervix because of its design. The 5-bladed Bouquet speculum gives a full circumferential view of the cervix which is very crucial in visualizing lesions in all four quadrants. The difficulty with lateral vaginal wall collapse that may obscure the view of the cervix is eliminated with the use of Bouquet speculum.
Of the 75 women screened using bouquet speculum, none reported discomfort with the insertion of the Bouquet speculum. The Bouquet speculum is superior to the 2-bladed speculum in that it provides better visualization of the cervix due to the number of blades that prevent the collapse of the lateral walls and the addition of the light source provide more illumination in the field during imaging. Therefore, the Bouquet speculum is very user friendly, less discomfort to patient and helps in obtaining better images of the cervix
